Physician Referrals
A primary care provider must refer patients on managed care plans. Self referrals are acceptable in certain circumstances. When making a physician referral, please complete a UW Medicine referral request (PDF).

All physicians referring to neurological surgery will be offered the option of obtaining a secured password and directions for accessing your patient's medical records online using U-Link, which provides online access to medical records.
